Patients on warfarin should disclose every supplement they take to their pharmacist and undergo regular INR monitoring, particularly after any change in their supplement or dietary routine.
SSRIs and NSAIDs: A Bleeding Risk Hidden in Plain Sight
Selective serotonin reuptake inhibitors (SSRIs) such as sertraline, fluoxetine, and escitalopram are among the most commonly prescribed medications in Canada. Non-steroidal anti-inflammatory drugs (NSAIDs) — including ibuprofen (Advil, Motrin) and naproxen (Aleve) — are purchased daily without a prescription at pharmacies across the country.
Used together, these two drug classes significantly increase the risk of gastrointestinal bleeding. SSRIs reduce the ability of platelets to aggregate; NSAIDs irritate the stomach lining and further impair platelet function. The combination creates a risk that is meaningfully greater than either drug alone — a fact that is not printed on the label of any ibuprofen bottle.
Patients on SSRIs who need pain relief should discuss alternatives with their pharmacist. Acetaminophen (Tylenol), used at appropriate doses, does not carry the same gastrointestinal bleeding risk and is generally a safer option for this population.
ACE Inhibitors and Potassium-Sparing Diuretics
ACE inhibitors (such as ramipril or lisinopril) are cornerstones of heart failure and hypertension management in Canada. When combined with potassium-sparing diuretics such as spironolactone, or with potassium supplements, they can cause hyperkalaemia — a dangerous elevation of potassium levels in the blood that can trigger life-threatening cardiac arrhythmias.

Antibiotics and Oral Contraceptives: A Persistent Myth — and a Real One

The evidence largely does not support a clinically significant interaction between most common antibiotics and combined oral contraceptives. However, rifampin — an antibiotic used in the treatment of tuberculosis and certain other infections — is a genuine and potent exception. Rifampin substantially reduces the effectiveness of hormonal contraceptives through the same liver enzyme pathway implicated in many drug interactions.
Patients being treated with rifampin should discuss alternative or additional contraceptive measures with their prescriber for the duration of treatment and for at least one month afterward.
Warning Signs That an Interaction May Be Occurring
Drug interactions do not always produce obvious or immediate symptoms. Some manifest subtly over days or weeks. Patients should be alert to unexplained changes such as:
- Unusual bruising or prolonged bleeding from minor cuts
- Sudden dizziness, confusion, or changes in heart rhythm
- Unexpected nausea, vomiting, or gastrointestinal discomfort after starting a new medication
- A worsening of a previously well-controlled condition (such as blood pressure rising despite consistent medication use)
- Excessive sedation or uncharacteristic fatigue
Any of these changes following the addition of a new medication or supplement warrants prompt contact with a pharmacist or physician.
How to Be a Proactive Advocate for Your Own Safety
The most effective tool in preventing drug interactions is a complete and current medication list. This list should include every prescription drug, over-the-counter product, vitamin, mineral, and herbal supplement you take regularly — along with doses and frequency. Carry this list to every medical appointment, every walk-in visit, and every pharmacy interaction.
When filling a new prescription, ask your pharmacist directly: Does this interact with anything else I am taking?
